Half-length portrait, the sitter stands against a plain background looking out to his left with a hand on his gown. The most striking aspect of his appearance is his immense baroque moustache cascading like a waterfall well below his chin. He is clad in sumptuous robes: the gown of the Royal College of Surgeons of England and the red and purple of his Pennsylvania honours.
